Emergency Medical Procedures
In order for school personnel to safely care for your child, the following is required:
1. Completed school health plan, signed by your health care provider
2. Medication authorization form, if needed
It is the responsibility of the parent/legal guardian to provide all prescribed medication, supplies and equipment to the school. All forms and medication must be updated yearly or if changes occur. If an emergency arises before receipt of the necessary items, school district policy and basic First Aid procedures will be followed. If you have any questions regarding these forms, please contact the front office.
Your prompt return of this information will assist us to respond to your child's needs. Please complete and return during your child's registration time or as soon as possible before the start of school.
